| Discussion and Research on Personalized Information Supporting Services for Self- health Management |

| Abstract:Combining with analysis on health risk factors and chronic diseases self-management related factors, the paper proposes the contents of personalized information supporting services for self- health management. It also introduces the realization of information service platform from platform design, technical support and running process, which is of a reference value for improving self- health management level and perfecting the modes of information services. |
