| Abstract:On the basis of healthy community mode, taking chronic disease management as the breakthrough and contract service as the starting point, the informatization support system of chronic disease contract management service under healthy community mode has been realized. The system promotes the implementation of major medical and health reform policies such as hierarchical diagnosis and treatment as well as signing family doctors, separation of hospitals and medicine, and reform of payment system, which is innovative. |
