The chronic disease self-management program within communities is to carry out by voice-robots and cloud platforms in this paper, including healthy education, consulting, health status evaluation, follow-up visit by robots and other functions, so as to achieve systematization and informatization of chronic disease self-management in communities and provide new methods and modes for chronic disease self-management.
