The paper analyzes the problems existed in current community health management, proposes a two-layer "personal-community" health management model. Through the technologies of the Internet of Things, real-time data acquisition, cloud computing, multi-network integration, it realizes continuous monitoring and intelligent management of health data quickly and effectively, the community health management goal is achieved.
