Combined with the national planning and guidance for health informatization, the paper discusses the construction of regional information platform for chronic diseases prevention and control based on electronic health records, introduces the constitution framework of the platform, elaborates the main functions, analyzes the key problems. Through data exchange and resource sharing, chronic diseases could be prevented and controlled.
