The paper introduces the characteristics of physical examination records, studies the data organization and collection method of health records based on physical examination, elaborates collection standard of data elements, introduces the composition of health records, including the basic information, illness history information, clinical physical examination information, laboratory examination, imaging examination information of the subjects, in the last puts forward the architecture of examination information collection and comprehensive management systems.
