The paper analyzes the current situation and problems of community health services, proposes to build the active health service platform for smart community and service evaluation index set, expounds the overall framework, service system, application scene and function design of the platform and the evaluation indicators of active health services, points out that it is helpful to realize the active and precise management of residents' health based on the platform.
